Leader training is a comprehensive certification process designed to equip individuals with the tools to facilitate evidence-based chronic disease self-management workshops. These programs rely on a peer-led model, meaning you don’t need to be a healthcare professional. During training, you will master the curriculum and learn essential facilitation skills, including how to guide group discussions, model action planning, and support participants in overcoming common challenges.

Chronic Disease Self-Management Leader Training
Oregon Wellness Network hosts no-cost virtual leader training for the SMRC evidence-based Chronic Disease Self-Management Program (CDSMP), originally developed at Stanford University. This training is ideal for LPNs, CNAs, social workers, and retirees (e.g., teachers and nurses). Participation prepares individuals to facilitate evidence-based classes in chronic disease self-management and serves as the basic building block for other cross-trainings.

Diabetes Self-Management Program Leader Cross-Training
OWN provides free leader cross-training in the SMRC model of diabetes self-management patient education. This training will prepare individuals to facilitate evidence-based classes in diabetes self-management. In addition, participants will learn OWN’s DSMES protocols.
The Diabetes Self-Management cross-training is available to individuals who have completed the 2020 Chronic Disease Self-Management Program (CDSMP) Leader training or update. Bilingual speakers (English and Spanish) may take this training in English and teach the class in Spanish (Programa de Manejo Personal de la Diabetes). If this describes your situation, let the master trainers know so that they can provide the manuals in English and in Spanish.

Additional Details
- If you are not currently a leader, you will be required to take the basic 4-day training in Chronic Disease Self-Management (CDSMP 2020) before being cross-trained in DSMP.
- Special DSMES Training on May 15, 2026 4:30-5:00pm. Open to all DSMP-trained individuals who are planning to facilitate any DSMP classes for OWN now and in the future.
- Class offerings include the most current manuals and textbooks (if you don’t already have).
